TeleVisit Patient Guide
TeleVisits can be scheduled by calling our front desk @651-241-5290. Please note that these visits are designed for our established and stable patients that live far away or have other significant hardships that prevent them from traveling to our clinic. Our most preferred method of seeing our patients is in person.
Unfortunately at this time we can not provide this service to Medicare patients due to stringent guidelines. If you are a Medicare patient and would like to schedule a TeleVisit, you must sign the acknowledgment of self-pay policy and the Medicare ABN form.
Please verify that you are in a state where your provider is licensed with the table below. We are actively working on getting our providers licensed in more states. If you need a prior authorization to see our specialists in person, then you will also need one to see them via telemedicine.
