Schedule an appointment

Either you, a family member or referring physician may schedule an appointment by calling our Patient Appointment Line at (651) 241-5290 between 8:00am and 4:30 pm. Monday through Friday.

When you call, please be prepared to provide the following information:

Patient’s full name, address, phone number, date of birth and social security number
Name, phone number and fax number of your primary care physician
Name, phone number and fax number of your referring physician if different from above
Insurance company and identification number, insurance phone number, policy holder’s name and name of employer
Name and number of a contact person

Some insurance carriers require that a patient obtain a referral from their primary care physician in order for the insurance to pay. If your insurance requires a referral, please contact your primary care physician’s office. They will submit a referral request to your insurance. You cannot assume that your referral has been approved unless you have received confirmation from your insurance company. If you are not sure if your referral has been approved, please contact your insurance company.

Parking

Surface Parking
Located behind the Nasseff Specialty Center – to enter the lot, turn onto Chestnut Street – the lot is on the South side of the street. If parking in the surface lot, present your parking ticket to the clinic receptionist – the ticket will be stamped and your fee when exiting the lot will be $2.00 – if it is not stamped, the exit fee will be $15.00.

Ramp Parking
The Gold Ramp is located at Smith and Chestnut, across the street from the Nasseff Specialty Center.

Parking Ramp Fees:

0 – 1 hour = $3.00
1 to 3 hours = $4.00

If parking in a ramp for more than three but less than eight hours, you may request a Discounted Parking Coupon from the clinic receptionist. Your fee when exiting the ramp will be $4.00. If exiting the ramp without a discount coupon, additional fees may apply.

United Serving Seniors Program

United serving Seniors gives United Hospital and your physician the opportunity to offer senior patients a special package of free and discounted services.

Free Parking
A Serving Seniors card entitles you to up to four hours free parking in any of the United Hospital ramps anytime you have an outpatient medical appointment in the Nasseff Specialty Center, Doctors Professional Building, St. Paul Heart and Lung Center, Ritchie Medical Plaza, Fort Road Medical Building or the hospital.

Free parking is not valid for visiting, attending classes or support group meetings.

Your Serving Seniors card is also good for up to four hours free valet parking when receiving services on the United Campus. Valet parking is available from 6:00 am to 6:00 pm Monday through Friday at the main entrance of the hospital and from noon to 10:00 pm at the Emergency Department entrance.
